The Omaha system nursing mode has many applications in the field of nursing, and its effectiveness has been confirmed in these applications. In this paper, we investigated the application of Omaha systematic nursing mode in transition nursing, community nursing and the elderly nursing. These surveys indicate Omaha system nursing mode has extensive research and application in postperative continuing nursing, community care and the elderly care, and has good performance, but in the preoperative transiton nursing has less application. The deficiency of preoperative nursing will lead to the discontinuity of family-community-hospital, and enable nursing service for patients cannot be continuous. Therefore, we can combine the medical data system and mobile network terminal, applying the Omaha system nursing mode in transition nursing for family and community, that is, based on the causes and characteristics of hip fracture in the elderly, applying the Omaha system nursing to intervene the preoperative continuing nursing such as nutrition guidance, fall prevention, individualized nursing for elderly women after menopause. We believe these will help to reduce the incidence of hip fractures in the elderly and readmission rate due to fall and other causes, provide guidance for application of living quality of patients after hip fracture in the elderly, and improve the quality of postoperative survival elderly hip fracture.
